    I hereby authorize The Farmers Bank, Frankfort, Indiana, to initiate a debit/credit entry to my account listed below and the financial institution named below, hereinafter called "Financial Institution," to debit/credit the same to such account.

    By checking the box below I acknowledge the following:

    • The origination of ACH transactions to my account must comply with the provisions of U.S. law.
    • I am an authorized owner/signer for the below mentioned account and can authorize the one-time debit to fund the new CD Account.
    • I am liable for any returned funds and the CD Account will be closed should the transaction be denied or returned from my Financial Institution to The Farmers Bank.
    • Should the CD Account be closed within the first 90 days of purchase, by you or The Farmers Bank, the funds will be returned via ACH Credit to the below mentioned account.

    This authorization is to remain in full force and effect until The Farmers Bank has received written notification from me of its termination in such time and manner as to afford The Farmers Bank and Financial Institution a reasonable opportunity to act on the request.
